A personal health record refers to health records an individual initiates and maintains. A Personal Health Record should offer comprehensive and accurate details about an individual's medical and health history. This information is gathered from various sources and authorized individuals should be able to access the information from the internet.
PHR is not the same as digital health record because the latter involves the use of software systems that are designed by healthcare providers. There are no laws that require people to store information about their health in PHRs but they are useful. The information stored in the record includes adverse reactions to drugs, allergies, hospitalizations, illnesses, surgeries family history, laboratory results and medications. The record also includes your name, date of birth, emergency contact and blood type. If you have had a physical recently, it should also be included in the record.
You control your own record but you can share it with other people including family members, providers and caregivers. The records are can be acquired from a variety of sources. Health plans usually have personal records for each member while some independent vendors can also store and receive information from pharmacies, providers and health plans.
